Lost PCard

  1. Please call US Bank at 800-344-5696 to report the card as lost.
  2. US Bank will need to verify recent charges with you to determine if the card was also compromised.
  3. You will need the last 4 digits of your WVU ID and the billing address (PO Box 6024, Morgantown, WV 26506) to verify the account.
  4. Once you have talked to US Bank, either the cardholder or the department will need to submit a PCard Application & Changes form in Mountaineer Marketplace to close the old card so we can release the new card once received.
